Bus Name
If the excessively smoking vehicle was a bus, select the name of the bus company from the list. If the vehicle was not a bus, then select not applicable from the list and make sure you provide the license plate number.

Bus Number
Enter the bus number, not the necessarily the bus's license number. If the bus was not numbered, please enter the bus license number.

Location
Enter the location you observed the excessively smoking vehicle. Same examples might be:

I80 Westbound near University Ave
US101 Northbound past Hwy92
El Camino Real South past 20th Ave
Treat Blvd East crossing The Bart Station

Caller ID
If you checked the "Yes" box , a confirmation letter will be sent to you when the District notifies the registered owner of the smoking vehicle reported.

In that letter you will be assigned a CallerID number. You may use this CallerID number for future reports. It is not mandatory that you use the same CallerID number each time you report. It is simply a convenience to aid you in completing the form.

Confirmation Letter
If you checked the "Yes", a confirmation letter will be sent to you when the District notifies the registered owner of the smoking vehicle reported.
